How
do I get started with physical
therapy treatment at ProActive?
An initial evaluation will need to be set up with one of
our licensed physical therapists. Patients should show up
about 15 minutes early for their first appointment to fill
out insurance information and past medical history
information. Patients can also pick up these sheets days
before their scheduled appointments and fill them out at
home. Patients should bring their prescriptions, health
insurance cards, related X-rays or MRIs, and any medical
forms or lists that would be pertinent to their care.

What will treatment consist
of with a physical therapist?
A patient will see a physical therapist for their initial
evaluation. The therapist will perform an evaluation and
will set up an individualized plan of care for the
patient. Treatments can include soft tissue work, specific
exercises, stretching, range of motion exercises, and
modalities (specialized machines to help improve
symptoms). The therapists work closely with referring
physicians to provide complete patient care.

How long do
physical therapy appointments normally last?
The initial appointment usually lasts 45 to 60 minutes.
Most physical therapy appointments will last 30 to 45
minutes. Supervised therapeutic exercise may also be
incorporated into the plan of care, and this will usually
take another 15 to 30 minutes.
We recommend that patients schedule ahead with our front
desk staff to ensure that they get in for the prescribed
number of appointments around times that are acceptable.

What should I wear
to physical therapy appointments?
A patient should wear comfortable clothes so that the
therapists can evaluate and treat the injured areas. The
patient should also wear something comfortable to work out
in. Physical therapy works intimately with the supervised
therapeutic exercise personnel to optimize independence,
strength, and function.

Will the physical therapist
prescribe any exercise?
ProActive aspires to not only improve specific
rehabilitative conditions, but also to improve the overall
health and activity patterns of each patient during
physical therapy treatment. ProActive continues a person’s
recovery in physical therapy by designing an exercise
prescription not only to rehabilitate the injury, but to
condition the entire body. The physical therapist will
also often prescribe take-home exercise to increase the
success of treatment.

Can I get a consultation
about physical therapy?
A free 15-minute consultation can be set up with a
physical therapist to determine if a patient needs
physical therapy. Our front desk staff will set up your
consultation.

Does ProActive do in-home physical therapy?
Yes. ProActive can provide in-home physical therapy services through many insurances, including Medicare. This allows a physical therapist to come to your home and help with functional activities until you are able to get to our facility safely.

Will my insurance
cover physical therapy treatment?
ProActive accepts most insurance plans (see list of companies below). Coverage varies based on individual policy plans. Some insurance plans require a physician’s prescription in order to receive physical therapy coverage benefits (ie. Medicare). You can check with your insurance plan to see if this applies. Please be aware that verification of physical therapy benefits is not a guarantee of payment. You are responsible for full payment of physical therapy services rendered at ProActive.
If you do not have health insurance and/or wish to not go through personal health insurance, you will be billed at a set self-pay rate. 45, or 60 minute self-pay appointments are available.
