Frequently Asked Questions

 

What do I need to bring for my initial therapy visit?

  • Identification card

  • Insurance card

  • ProActive PT paperwork (which can be found under the 'Patient Resources' tab)

  • Your prescription, if applicable.

  • If you have a Worker's Compensation case, please bring your claim number.

  • Comfortable clothing for movements and therapy testing.

What can I expect for my initial therapy visit?

  • You will meet one-on-one with a physical therapist who will talk with you about your scenario and goals.

  • The therapist will assess your positioning and movements.

  • The therapist may use hands on techniques and individualized exercises to re-educate your movements.

  • The therapist will spend time teaching you about your unique scenario, educate you on improving your present movements, and help you to prevent future injuries.

What do I need to know for follow up visits?

  • You will work with your same therapist for subsequent visits unless you request someone different.

  • Your therapist will review and adjust your treatment plan based on your progress.

  • Appointments last approximately 45 minutes to 1 hour.

How do appointments work?

  • Plan on arriving a couple minutes early for your appointment.

  • Co-pays are paid at the time of service.

  • Many times, we will get you warmed up with some supervised exercises that the therapist prescribes.

  • After your appointment, we will review future appointment times and we will give you an appointment card as a reminder. We can sign you up for email reminders as well.

  • If you are going to be late or if you are going to miss your appointment, please call us ASAP so that we may reschedule you.

What Insurances do you take?

  • We take many insurances, including Medicare, Anthem Blue Cross Blue Shield, United Healthcare, TriWest, and Simplified Benefit Administrators (PSD).
     

    • We are in-network with other insurance companies that may not be listed.

    • Furthermore, we can be deemed out-of-network with certain exclusive plans. Please verify this with your insurance provider. 
       

  • We are NOT Medicaid, Cigna, Aetna, Humana or Kaiser Permanente insurance providers - we do not accept these as primary insurance.  Medicare supplement (secondary) plans with these insurances are accepted. If we do not accept your insurance, you may still be treated at our facility at our self pay rate.
     

    • We are out-of-network with other insurance companies that may not be listed, so give us a call to confirm if we may be able to accept your insurance.
       

  • Please call your insurance provider to double check on your specific plan and be aware of your specific physical therapy coverage. You will be financially responsible for any denied claims or for any claims that could not be processed due to incorrect and/or a lack of current insurance information.
     

  • Please inform us of any changes to your insurance, personal address, referring physician, or any other important information related to your care.
